We, as KÖKSAN
PET VE PLASTİK AMBALAJ SANAYİ VE TİCARET ANONİM ŞİRKETİ (hereinafter
referred to as the "Company"),
inform you that we process your personal data in the capacity of data
controller within the scope of the Law No. 6698 on the Protection of Personal
Data (hereinafter referred to as "KVKK")
and other relevant legislation.

The categories and descriptions of personal data to be
processed within the scope of the purposes and legal reasons specified in this
Disclosure Statement for Employee/ Trainee Candidates in accordance with the
KVKK and other relevant legislation are as follows:
Identity Information: Name-surname,
place and date of birth, nationality, mother's name, father's name, gender,
driver's license.
Contact Information: Home
and mobile phone number, e-mail address, address.
Education Information: Information
on education, foreign language skills, computer skills, work experience,
training/course attended.
Personnel Information: Information on military service
status
Sensitive Personal Data: Photograph, blood type.
Other: Height-weight
information, Marital status, Smoking information, Signature
c) Purpose for which
Personal Data will be Processed
Your personal data will be processed for the following
reasons;
- Managing employee/intern candidate selection and
placement processes,
- Carrying out the application processes of
employee/intern candidates,
- Planning of human resources processes,
- Fulfilment of obligations arising from the labour
contract and legislation,
- Carrying out communication activities,
As stated in Article
5 (2) of the KVKK "a) "c) It is necessary to process personal data
belonging to the parties to the contract, provided that it is directly related
to the establishment or performance of a contract", "d) It is
mandatory for the data controller to fulfil its legal obligation". Your
sensitive personal data (passport size photograph) will be processed by
obtaining your explicit consent based on the legal reason specified in Article
6 (2) of the KVKK for the purpose of planning human
resources processes.
d) Method and Legal
Grounds for Collection of Personal Data
In order to fulfil the purposes specified in paragraph
(c) of this text, your personal data will be obtained verbally, in writing or
electronically by the following automatic or non-automatic methods, based on
the legal grounds in Article 5 (2) of the KVKK:
- Filling in the job application form or providing
information during the job interview.
- Submission of a CV by the employee/trainee
candidate.
- Transmission of information by the person referred.
e) For what purpose the
processed personal data can be transferred
Within the scope of the purposes specified in
paragraph (c) of this text, your personal data will be transferred by obtaining
the explicit consent of the employee candidate in accordance with Article 8 (1)
of the KVKK in order to be evaluated in appropriate positions, if necessary or
upon request.
f) Your Rights under
Article 11 of the KVKK
Within the scope of Article 11 of the KVKK, you have
rights as a data subject and you can submit your requests regarding these
rights by filling in all the information specified in the Relevant Person
Application Form in accordance with Article 11 and paragraph 1 of Article 13 of
the KVKK and the Communiqué on the Procedures and Principles of Application to
the Data Controller,

registered in our systems or by other methods to be determined by the Board in
the future.
In addition to the
rights specified in Article 11 of the KVKK, if you give explicit consent to the
processing of your personal data, you have the right to withdraw such explicit
consent at any time.
About The Processing of Personal Data Statement of Explicit Consent
KÖKSAN PET VE PLASTİK AMBALAJ SANAYİ VE TİCARET
ANONİM ŞİRKETİ (hereinafter referred to as “KÖKSAN”), that I have read and
understood the Clarification Text submitted to me in accordance with the
relevant provisions of the Law on the Protection of Personal Data No. 6698
(“KVKK”). I accept and hereby declare that the clarification text has been
provided to me.
In this context, due to the job application I
have made at KÖKSAN, my personal data regarding the following categories are
used by KÖKSAN or third parties for the purpose of carrying out business and
service processes in accordance with the privacy policies, evaluating potential
business opportunities, fulfilling the work and transactions that must be done
in line with the obligations of KÖKSAN within the scope of the relevant
legislation. I voluntarily accept and declare that I have my express consent to
be processed.
With this Statement of Explicit Consent, all my
personal data, including my sensitive personal data, and the related personal
data that will be processed/stored after my express consent is given, will be
processed and stored for 5 years from the signature date of this Explicit
Consent Statement, within the scope of the relevant process. I accept that it
is used limited to the purpose of processing and that it is shared with
domestic and foreign third parties and group companies and service providers
inside and outside the company, transferred to these persons and stored on
servers located abroad, hence, upon my open and free will I accept and declare
that I express my explicit consent within the scope of the relevant provisions
of the KVKK.
In addition to the above issues, I declare that I
am aware of the following rights regarding myself within the scope of the Law:
a. Learning whether my personal data is processed
or not,
b. If my personal data has been processed,
requesting information about it,
c. Learning the purpose of processing my personal
data and whether they are used in accordance with the purpose,
d. Knowing the third parties to whom my personal
data is transferred, in the country or abroad,
to. Requesting correction of my personal data if
it is incomplete or incorrectly processed,
f. Requesting the deletion or destruction of my
personal data within the framework of the conditions stipulated in the Law,
g. Requesting notification of the transactions
made pursuant to articles (e) and (f) to the third parties to whom my personal
data has been transferred,
h. Objecting to the emergence of a result against
me by analyzing my processed data exclusively through automated systems,

suffer damage due to unlawful processing of my personal data.
